Work health and safety forms and fees

Forms will need to be lodged with the appropriate fee, either in person, by post or via the online registry. Find out how to lodge a form or pay a fee. 

These fees are effective from 1 July 2025. 

Fees for criminal cases brought under the Work Health and Safety Act 2011 are prescribed by the Criminal Procedure Regulation 2010.
